Nethermost Pike rises to 891 m (2,923 ft) on the main Helvellyn ridge between Helvellyn and Dollywaggon Pike. It is a Wainwright, Nuttall and Birkett, with 29 m of prominence. Helvellyn, the nearest higher fell, is 0.9 km to the north, and Dollywaggon Pike 1.2 km to the south.
Most walkers reach it on the ridge from Helvellyn or from Grisedale Tarn, and the main path skirts the summit to the west. From Wythburn by Thirlmere, a path climbs to the ridge between Nethermost Pike and Helvellyn. From Patterdale, the east ridge rises above Grisedale and Nethermost Cove, a steeper and quieter line.
The summit is a broad, flat top with a cairn a short way east of the main path. The eastern edge drops in crags into Nethermost Cove and Ruthwaite Cove, and in winter cornices form along it.
